**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview and the facility's policy titled, Medication Storage Storage of Medications and Biologicals, the facility failed to ensure an opened 5 ml (milliliter) vial of influenza vaccine with the expiration date of [DATE], and twelve unopened suppositories with the expiration date of 05/19, were not stored in the medication storage room refrigerator on [DATE]. This affected one of one medication storage room refrigerators observed. Findings include: Review of facility's policy titled, Medication Storage Storage of Medications and Biologicals, with a date of 01/12, revealed: .11. Outdated .medications .are removed from stock, disposed of according to procedures for medication disposal . On [DATE] at 9:19 a.m., the Surveyor observed the medication storage room with Employee Identifier (EI) #1, Registered Nurse (RN). The refrigerator in the medication storage room contained the following: 1. an open 5 ml multi dose vial of Influenza vaccine with an expiration date of [DATE] and 2. 12 Anucort -hc 25 mg (milligram) suppositories with an expiration date of 5/19. On [DATE] at 9:58 a.m., the Surveyor conducted an interview with EI #1. EI #1 was asked, did she observe an open vial of influenza vaccine that was expired in the refrigerator. EI #1 replied, yes. EI #1 was asked what was the expiration date. EI #1 replied, she thought it was [DATE]. EI #1 was asked where should the expired open vial of influenza vaccine be stored. EI #1 replied, it should have been placed in the destruction container. EI #1 was asked, did she also observe twelve Anucort suppositories that were expired in the refrigerator. EI #1 replied, yes and the expiration date was 05/19. EI #1 was asked, what was the potential harm with having expired medication stored in the medication storage room refrigerator. EI #1 replied, the resident could have received the medication if someone did not check the expiration date before they gave the medication.
