**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review and staff interviews, the facility failed to develop a baseline care plan for one of two residents (R#353), who required treatment for a UTI (Urinary Tract Infections). Finding include: Review of the facility policy titled, Policy titled Care Plans-Preliminary (undated) documented A preliminary plan of care to meet the resident 's immediate needs shall be developed for each resident within forty-eight (48) hours of admissions. Review of R#353 's face sheet documented an original admit date of 9/22/2021. Further record review revealed that R#353 was sent out to the hospital on 9/27/2021 due to complications of low sodium. R#353 was re-admitted to the facility on [DATE] with a diagnosis of urinary tract infection (onset 9/27/21) Review of document titled, admission Nursing Assessment V 3 documented under section Admitting Diagnosis 1. Reason for admission to nursing facility-readmitted to the facility after hospitalization for Hyponatremia, Anemia, and UTI. The document documented under section Assessment Resident readmitted to the facility after hospitalization, Dx (diagnoses) of hyponatremia, anemia and UTI. Review of R#353's baseline care plan revealed that there was no evidence of a baseline care plan in the resident's record. In addition, review revealed that there was not a baseline plan of care to address the resident diagnosis of a UTI (urinary tract infection) and the treatment steps for a urinary tract infection. Review of the Medication Administration Record (MAR) for R#353 dated 10/1/2021 revealed an order dated 10/1/2021 for the following medication: Cefdinir Capsule 300 MG Give 1 capsule by mouth two times a day for infection until 10/05/2021 20:59. Review of a nurse note for R#353 regarding treatment for UTI documented 10/2/2021 10:53 Infection Note Text: The resident is on ABT (antibiotic) cefdinir 300 mg (Cefdinir 300 mg) for uti (UTI). The resident has no signs or symptoms of adverse reaction noted. The resident is encouraged to drink fluids throughout the shift. Interview on 10/6/2021 at 10:49 a.m., the Minimum Data Set (MDS) Coordinator confirmed and verified that R#353 did not have a baseline care plan to address her UTI (urinary tract infection). She also stated that the nurse failed to create a baseline care plan after the resident returned (as a newly re-admitted resident) from the hospital. The MDS Coordinator reported that any staff can create a baseline care plan in the system. The MDS Coordinator described the system protocol is that the baseline care plan should be created the same day or no later than the next day (after a resident admission to the facility). Interview on 10/7/2021 at 2:05 p.m., the Director of Nursing (DON) revealed that her expectation is that her nursing staff create a baseline care plan.
