**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview and record review, the facility failed to ensure that the emergency call light system in resident bathrooms were functioning properly for four of eight resident bathrooms (Rooms 322/324, 318/320, 313/315, and 311) on the secured memory unit. Findings include: On 9/23/22 at 10:36 a.m., the call light for the conjoined resident bathroom in room [ROOM NUMBER]/324 was tested. When pulled, the call system did not activate. Certified Nursing Assistant (CNA) AA confirmed that the call light was not functioning. On 9/23/22 at 10:45 a.m., the call light for the conjoined resident bathroom in room [ROOM NUMBER]/320 was tested. When pulled, the call system did not activate. CNA AA confirmed that the call light was not functioning. On 9/23/22 at 10:48 a.m., the call light for the conjoined resident bathroom in room [ROOM NUMBER]/315 was tested. When pulled, the call system did not activate. CNA AA confirmed that the call light was not functioning. On 9/23/22 at 10:50 a.m., the call light in the resident bathroom was tested in room [ROOM NUMBER]. When pulled, the call system did not activate. CNA AA confirmed that the call light was not functioning. During an interview on 9/23/22 at 11:04 a.m., the Safety Officer stated that the maintenance director was out, and he is covering in their absence. He stated that they do daily audits by choosing three rooms a day on each hall to check for maintenance issues and that is to include ensuring the bathroom call light function properly. He confirmed that the cords were too tight and were not functioning as intended in the resident bathrooms (Rooms 322/324, 318/320, 313/315, and 311). During an interview on 9/25/22 at 11:30 a.m., the Administrator, she stated that the expectation is that the call light cords should not be hard to pull to activate the system. She stated that she discussed with maintenance for them to routinely treat the bathroom call light cords during routine rounds to ensure that they do not become too hard to pull to activate the system. A review of the Nursing Call System dated 3/30/22 revealed that it is the policy of (the facility) to have an established nursing call system to ensure that residents can effectively communicate their needs timely.
