Based on record review and interviews the facility failed to electronically submit payroll information for direct care staffing as required. Findings: Review of the facility's Payroll Base Journal (PBJ) Staffing Data Report for Fiscal Year Quarter 2 2023 (January 1-March 31) revealed, in part, the facility failed to submit staffing data for the quarter. On 06/19/2023 at 12:48 p.m., an interview was conducted with S2DON. She stated in February she received an email from the Medicare Quality Improvement Organization concerning PBJ submissions. She stated when the responsibility was moved from Human Resources, the facility stopped submitting the PBJ report. She confirmed the PBJ report was not submitted for Quarter 2 of 2023 (January- March) and should have been.
