Based on observations, staff interviews, and review of the facility policies titled, Date marking for Food Safety and Food Safety Requirement, the facility failed to ensure that opened food items were labeled and dated. This deficient practice had the potential to affect all 54 of 54 residents receiving an oral diet. Findings included: Review of the facility policy titled, Date Marking for Food Safety, dated 5/1/2023, under subtitle, Policy Explanation and Compliance Guidelines for Staffing revealed: 2. The food shall be clearly marked to indicate the date or day by which the food shall be consumed or discarded. 3. The individual opening or preparing a food shall be responsible for date marking the food at the time the food is opened or prepared. Review of the facility policy titled, Food Safety Requirements, dated 5/1/2023, under subtitle, Policy Explanation and Compliance Guidelines revealed: 3. (c) Refrigerated storage-foods that require refrigeration shall be frigerated immediately upon receipt or place in freezer, whichever is applicable. Practices to maintain safe refrigerated storage include: (iv.) Labeling, dating, and monitoring refrigerated food, including but not limited to leftovers, soi t is used b its use by date, or frozen (where applicable)/discarded; and (v.) keeping food covered or in tight containers. Observation on 10/13/2023 at 8:04 a.m. of the walk-in cooler revealed the following: A plastic 20-ounce jar of Smucker's Sundae Carmel Syrup was not labeled or dated. A bag of Roseli mozzarella cheese, no weight provided. The bag was opened, unlabeled, and undated. Observation of the walk-freezer on 10/13/2023 at 8:08 a.m. revealed the following: A box of JBB Tender Shaped Chicken Breasts. Inside the box was a plastic bag that was open to the air. The bag was unlabeled and undated. A box of Chef's Line vegetarian hamburger patties. Inside the box was a bag of hamburger patties open to the air. The bag was unlabeled and undated. A bag of vegetarian breaded nuggets, no weight provided. The bag was open to the air and unlabeled and undated. A box of Supreme fish filets. Inside the box was a bag of fish filets, open to the air and unlabeled and undated. A box of Cattleman's Beef Patties. Inside the box was a bag of patties, which was open to the air, unlabeled, and undated. A bag of tater tots, no weight provided. The bag was open to the air and unlabeled and undated. A bag of waffles with four waffles. The bag was open to the air and unlabeled and undated. Interview on 10/13/2023 at 8:14 a.m. with the Director of Nursing (DON) acknowledged that staff did not properly store the items in the refrigerator and freezer. DON stated that staff should have sealed, labeled, and dated the food items. Observation on 10/13/2023 at 8:15 a.m. of the dried goods revealed an open bag of Roseli Tri Colored Pasta with no weight provided. The bag was unlabeled and undated. Interview on 10/13/2023 at 8:20 a.m. with [NAME] (AA) confirmed the items in the walk-in freezer, refrigerator, and dry goods were not stored properly, labeled, or dated after opening. He stated all items should have been stored securely, labeled, and dated. Interview on 10/14/2023 at 12:42 p.m. with the Dietary Manager (DM) acknowledged the staff did not store the items properly. DM stated the staff should have sealed, labeled, and dated the food items.
