**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on medical record review and facility staff interview, the facility failed to ensure a resident's advance directive status was consistent on all forms of the medical record. This affected one resident (#8) of 16 reviewed for advanced directives. The facility census was 27. Findings include: Medical record review revealed Resident # 8 was admitted to the facility on [DATE] with diagnoses including osteoarthritis, fracture of right pubis, hypertension, and type two diabetes mellitus. Review of the admission Minimum Data Set (MDS) assessment dated [DATE] revealed the resident had intact cognition. Review of the resident's 1/2019 physician's orders in the electronic health record and hard chart revealed the resident's code status was full code. Further review of the hard chart revealed an Ohio Do Not Resuscitate (DNR) Identification form signed and dated 01/09/19 by the resident's physician. The form documented the resident's code status as Do Not Resuscitate Comfort Care (DNRCC). Interview on 01/22/19 at 5:35 P.M., with the Director of Nursing (DON) verified the physician's orders in the electronic health record and on the paper physician's orders sheet in the hard chart documented the resident's code status as full code and did not match the Ohio recognized DNR Identification form signed and dated by the physician 01/09/19 for DNRCC code status.
